Intercollegiate Athletic Eligibility Requirements

The National Collegiate Athletic Association (NCAA) mandates that all potential student athletes, freshman and transfers have a current Medical Evaluation Form on file prior to participation in intercollegiate athletics. The medical evaluation (pre-entrance physical) form must be completed by a physician within six months of your arrival on campus and returned to the Health Center.

In addition, every student athlete must have proof of health insurance that provides coverage for their participation in intercollegiate athletics up to 75,000. The The College at Brockport student health insurance policy covers athletic related injuries up to $75,000. A copy of your medical insurance card, front and back must be on file with the Office of Intercollegiate Athletics prior to your participation.

Mandatory forms that need to be completed and returned to the respective office prior to the NCAA eligibility meeting are as follows:

RETURNING STUDENT-ATHLETES:
1) Returning Athlete History Form
2) Verification of Insurance Form and a copy of your insurance card
3) Consent for Release Form

NEW STUDENT-ATHLETES:
1) Medical Evaluation Form – FAX to the Health Center (585) 395-2559

The following forms should be returned to:

2) New Athlete Health History Form
3) Verification of Insurance Form and a copy of your insurance card
4) Consent for Release Form
