The Student Health Center is part of the Hazen Center for Integrated Care, which provides a holistic approach to student health and wellness. When the Health Center is closed
You may utilize our after-hours nurse triage line by calling (585)-395-2414 for medical advice, and following the prompts to connect to FONEMED. Registered nurses are available to talk with students to help determine a plan of care.
FONEMED is intended for medical advice only. Please note that they are NOT located on campus and do not have access to your personal medical record. Therefore, they cannot tell you your immunization compliance, call in prescriptions, or provide your lab or X-ray results.
A summary of each call will be faxed to the Student Health Center to ensure proper follow-up care.
Access MY HAZEN
The online 'Patient Portal,' MY HAZEN, makes it easier for you to complete forms and send Secure Messages.
The Patient Portal and Secure Messages should be used for non-urgent medical questions, lab results, routine follow-up questions, etc.
DO NOT use the Secure Messaging to communicate an emergency or urgent issue or to request an appointment.
All communication via the Patient Portal will become part of your permanent medical record.
Please go to MY HAZEN to sign in using your NetID and password. Your NetID is your Brockport email without the @brockport.edu (e.g. if your email
is email@example.com, your NetID is jdoe1). Your password is the same password you
use for your Brockport email.

With the exception of emergency situations in which a student is unable to assume responsibility, all medical information is confidential. The Student Health Center will not share information concerning the health of a student without the student's explicit and informed written authorization. Notification of parents, College faculty, and administrators is considered the student's responsibility.
